How to File a Claim

A claim should be lodged with the following documents:

  • Completely filled Claim Form duly signed and stamped by the attending physician./li>
  • Original itemized Hospital Bill (in case of hospitalization)
  • Doctor’s Prescriptions (photocopy acceptable)
  • Laboratory reports (photocopy acceptable)
  • Original Pharmacy payment receipts
  • Discharge summary (photocopy acceptable)
  • Birth Certificate (in case of delivery)

Our claims team will review the documents and approve the claim. If no further information is required and the treatment taken is covered as per the policy terms and conditions, Allianz EFU will settle the claim. Payment in all such cases will be made within 15 working days of the receipt of the claims.

In case of any query or incomplete documentation, a requirement letter will be forwarded to the insured / client. Payment will be made within 15 working days of the receipt of all required documents.

If the claim is not payable as per the policy terms and conditions then a regret letter will be forwarded indicating the reason for non-payment.
